The Hill Park Approach to GLP-1 Therapy

We have been supporting our patients’ use of GLP-1 receptor agonist medications such as Tirzepatide and Semaglutide for nearly two years and have acquired key clinical insights on the safest, most effective, and sustainable approach to using these medicines.

The Basics of GLP- 1 Medications:
GLP-1 receptor agonists mimic glucagon-like peptide-1, a natural hormone that regulates blood sugar and appetite. Originally designed to treat Type 2 Diabetes, they are now widely used for weight loss due to their effects on insulin, fat storage, delayed gastric emptying to keep you full longer, and signaling the brain to reduce cravings and “food noise”.

The elimination of “food noise”, which can especially be in the form of emotional eating or “thoughtless” eating, has been remarkably liberating for many of our patients, but especially women in peri-menopause/menopause that tend to have hormone driven cravings.

Emerging research on GLP-1–based therapies continues to expand beyond weight management, with potential benefits in areas such as metabolic health, inflammation reduction, support for neurocognitive conditions such as dementia and Alzheimer’s, addiction behaviors (including food, alcohol, and gambling), and post-viral syndromes such as Long COVID and chronic fatigue syndrome (CIRS).

Low Dose Tirzepatide
We are seeing uniquely remarkable results with low dose Tirzepatide in Mast Cell Activation Syndrome (MCAS) and chronic pain and inflammation due to complex chronic illnesses such as Tick-Borne Illness and endometriosis.

Longevity Impact of GLP-1 Outcomes
The cardiometabolic effects are profound; it is very common that we are reducing or eliminating statin use, metformin dependence, augmenting medications that have been controlling metabolic health once the body adjusts to its new weight, new metabolism, and greater cellular health.

The health effects of dramatic improvement of lipid profiles, hypertension, and glucose management are profound enough that research indicates this use across most specialties, but especially oncology for reduction of remission risk by dramatic improvement of health.

These therapies can support healthy weight loss, reduce visceral fat, and help regulate cravings; however, long-term success depends on integrating sustainable lifestyle changes. Our practitioners are committed to guiding, educating, and supporting you throughout the process to ensure both safety and long-term success.

Troubleshooting the Weight Loss Plateau
A GLP-1 weight loss plateau can actually be a normal phase of our body’s adjustment to a new metabolic baseline with weight loss; it’s not necessarily a failure, but a time to pivot the plan. The insight of clinical expertise can help to walk you through workable solutions, returning to forward momentum.

Additionally, some folks have encountered challenges throughout the use of these medications on their own or with online services. Our unique, individualized approach allows our patients to navigate their journey sustainably.

Let’s begin the journey!
To get started, we would set you up with a brief 30-minute intake appointment with Dr. Gabrielle Klein, who leads our Tirzepatide program. At this visit she will start with comprehensive baseline lab work to assess your overall health and inform your treatment plan.

She will also discuss the complex drivers of weight gain and how they can impact your weight loss journey by assessing and discussing the phenotypes that embody them: metabolic dysfunction (PCOS, Insulin Resistance, T2DM), the hungry brain (food noise, constant preoccupation with eating), the hungry gut (large meal capacity and difficulty feeling full quickly), the hedonistic eater (stress eating, rewards-driven eating), or a combination.

Ongoing care includes regular check-ins to monitor progress, track body composition, manage any side effects, and quarterly reassessment of labs to track physiological progress. Additionally, we bring on nutritional support to protect lean muscle and bone mass, minimize the medication’s side effects, and ensure you are meeting your goals.

If this sounds like the right direction for you, or you would like to learn more about the process, you may email or call Arlene at 707-861-7300, or schedule online.

Welcome Dr. Brandi Holmes, ND to Hill Park Integrative Medical Center

We are very excited to announce a new addition to our medical team, Dr. Brandi Holmes, ND.

Dr. Brandi Holmes is a licensed naturopathic doctor in California and Arizona specializing in personalized, integrative care for active women and athletes. With a passion for optimizing performance, preventing injury, and restoring balance, Dr. Holmes offers a holistic approach that connects the dots between hormones, musculoskeletal health, nutrition, and mindset. Her mission is to empower each patient to feel strong, energized, and in sync—on and off the field.

A lifelong athlete, Dr. Holmes began competing in kindergarten and continued through college with a primary focus in track and field, specializing in sprints and hurdles. She also played softball, lacrosse, tennis, and trained in martial arts. Her competitive drive and “no pain, no gain” mindset led to athletic success—but also a cycle of repetitive injuries, including shin splints, stress fractures, and ankle sprains. These experiences sparked her early interest in injury prevention and whole-body wellness.

Dr. Holmes pursued dual undergraduate degrees in Kinesiology (with a focus on physical therapy) and Cellular & Molecular Biology at San Diego State University. During this time, she gained five years of hands-on clinical experience at a physical therapy practice, where she developed skills in rehabilitative therapies such as aquatic therapy, Pilates, electrotherapeutic modalities, and functional movement assessments.

Her desire to practice integrative medicine led her to Bastyr University California in San Diego, where she earned her Doctorate in Naturopathic Medicine. Her training included botanical medicine, nutritional therapy, targeted supplementation, pharmaceuticals, and lifestyle counseling, as well as advanced physical medicine techniques including soft tissue manipulation, therapeutic exercise, and electrotherapy.

Dr. Holmes takes a systems-based approach to sports and performance medicine, recognizing that true recovery and peak performance require a deeper understanding of how all organ systems interact with the musculoskeletal system. She is especially passionate about supporting hormonal balance, gut health, and metabolic optimization in active individuals.

Dr. Holmes partners with patients to create personalized, evidence-informed plans that support both short-term recovery and long-term vitality. Whether you’re an athlete striving for peak performance or someone ready to feel at home in your body again, Dr. Holmes is here to help you thrive—every step of the way.

“Optimal athletic performance isn’t just about muscle strength and flexibility—it’s a symphony where every body system plays a part, from digestion fueling endurance to hormones fine-tuning recovery. When all systems work in harmony, athletes unlock their true potential.”

Dr. Holmes is currently accepting new patients.

How IV Vitamin Therapy Can Help to Improve Your Health

How IV Vitamin Therapy Can Help to Improve Your Health

If you knew about a single vitamin that not only helps give you more energy, but helps you sleep more soundly; that increases cognition, but also calms your mind, would you take it? Especially knowing this vitamin is very safe and almost impossible to have too much of? This vitamin can do all of the above, along with helps heal nerves, elevates your mood, supports liver health, and more. Which vitamin is this?

Vitamin B12, or methylcobalamin, is found naturally in meat, eggs, and dairy, but also, in sea vegetables, chlorella, spirulina, and brewer’s yeast. It can only be absorbed orally if there is sufficient stomach acid, so if you are on any medications like Prilosec or Prevacid, you might not be absorbing it well. Other medications can also deplete B12 like metformin, statins, and diuretics. Even other conditions like celiac disease or GI inflammation can inhibit the absorption.

B12 can help with DNA production, red blood cell health and production, nerve pain, energy, and even herpes viruses like cold sores and shingles. It is used by almost every cell of the body and helps with energy production, pain, and healing. When people are deficient in B12, which can be a result of pernicious anemia or chronic stress, they often experience nerve and muscle pain, brain fog, difficulty sleeping, depression, immune dysfunction, and skin issues. Naturopathic doctors have been using this vitamin for a very long time to increase healing time, elevate mood, and increase energy.

Hill Park Integrative Medical Center recommends B12 in its methylcobalamin or hydroxocobalamin form and as an injection. This is to ensure the best absorption of B12, as injections allow you to absorb 100% of the dose since, it goes directly into the bloodstream.

We provide quick, painless injections on a walk-in basis. Most people come in once a week for four weeks in a row and report feeling a profound difference in their energy, mental clarity, and overall wellbeing. Hill Park is also offering walk-in B12 Happy Hours featuring a short consultation with one of our functional medicine doctors to help you choose which nutrient injection is best for you! They are Monday 3-6pm, Wednesday 4-6 pm, and the first Saturday of every month (Super Saturday!) from 10am-1pm.

B12 is also found in many of our IV and customized nutrient formulations, so give us a call or walk on in to give it a try!
